Human-in-the-loop, done right

Approval steps can make agents safe — or make them useless. Five patterns for oversight that doesn’t slow you down.

Marcus Bell

Head of Delivery

Every client asks the same question in the first meeting: what happens when the agent gets it wrong? The honest answer is that it will, sometimes — and the system around it decides whether that matters.

1. Approve by exception

Don’t ask humans to approve everything. Ask them to approve what the agent is unsure about, what’s high-value, or what’s irreversible.

2. Show the reasoning

An approval request should include what the agent saw, what it decided and why. Reviewers approve faster when they don’t have to reconstruct context.

3. Make undo easy

Every action should be logged and reversible for a window of time. Reversibility is what lets you move fast.

4. Review a sample, always

Even fully automated flows deserve a weekly sample review. It’s how you catch drift before customers do.

5. Close the loop

When a human corrects the agent, that correction should become a test case. Over time, the agent earns autonomy one category at a time.
